RESUMO

29 patients aged 6-16 with glomerulonephritis lasting 4-5 years received multimodality treatment with plasmapheresis as a component. The majority of the patients suffered from primary glomerulonephritis in mesangio- or membrano-proliferative morphological variants. Previous long-term conventional therapy (prednisolone, cytostatics, anticoagulants and antiaggregation drugs) failed. The test course comprised 1-3 plasmapheresis sessions (centrifuge method on [symbol: see text] apparatus), cyclophosphamide or maintenance methyl-prednisolone pulse therapy, heparin and curantil. One-third of the patients achieved remission lasting from 5 months to 3 years, in the other one-third the improvement was as short as 2-4 weeks, and the last one-third appeared non-responders. Improvement of clinical indices occurred in parallel with trends to reduction in the levels of CIC, IgG, B-lymphocytes, T-helpers, inhibition of lymphocyte succinate dehydrogenase activity, better phagocytosis. No complications which may prohibit plasmapheresis use in glomerulonephritis were observed. Adjuvant plasmapheresis use in glomerulonephritis treatment needs further studies.

RESUMO

Fifty-seven children aged 1 to 14 years with pseudoallergic reactions were followed up. The reactions manifested in the form of eczema, neurodermatitis, urticaria, and Quincke's edema. All the patients showed different pathology associated with the above-indicated skin reactions. The infants presented with the hypertensive hydrocephalic syndrome whereas the elder children with vegetovascular dystonia, biliary and gastrointestinal diseases. The treatment of the revealed pathological process, that included administration of histaglobulin or zaditen, brought about well-defined positive clinical changes.

As many as 58 children aged 3 to 14 years operated on for destructive forms of appendicitis complicated by diffuse peritonitis were examined for microcirculation of eye conjunctiva microvessels using biomicroscopy and for chemotactic++ activity of macrophages in an area of aseptic inflammatory reaction. The correlation analysis made it possible to reveal specific dependence of macrophagal chemotaxis on primary derangement in one or another component of the microcirculatory bed, the extent of diminution of the arteriolar, capillary and venular lumen as well as on the impairment of blood aggregation in microvessels. Detoxication therapy contributed to the improvement of microcirculation and the growth of the release of macrophages to the area of aseptic inflammatory reaction.
